Hundreds of occasional operators, cascade training and no register: why competency has to be recorded per person, per device and per test.

Control results and EQA returns that are visible across every site, and a loop that closes the failures that would otherwise repeat.
Results that reach the record
A result on a device screen or in a photographed log is not yet a result the service can govern. What changes when it lands where it belongs.

One view across sites, devices and operators, so that a drift is noticed on a Tuesday and not at the annual assessment.
